At Community First Services and Supports (CFSS), we are committed to empowering individuals with disabilities, seniors, and those with special healthcare needs by providing flexible, person-centered services. Our goal is to enhance independence, allowing participants to live more fulfilling lives in their homes and communities across Minnesota.

What is CFSS?

CFSS is a unique program designed to meet the diverse needs of Minnesota Health Care Programs (MHCP) members. Through CFSS, individuals gain greater control over their care, ensuring they receive the support they need, when and where they need it. Whether at home or in the community, CFSS supports a wide range of activities to help people maintain their independence.

Effective October 1, 2024, CFSS will replace Personal Care Assistance (PCA) and the Consumer Support Grant (CSG). PCA and CSG will be phased out, and CFSS will take on these critical services.

Who is Eligible for CFSS?

CFSS is available to members of various Minnesota Health Care Programs, including:

  • Alternative Care Program (AC)
  • Emergency Medical Assistance (EH)
  • MinnesotaCare (children 18 years and younger and pregnant women)
  • Medical Assistance (MA)
  • Refugee Medical Assistance (RM)

Our Services

CFSS services include a variety of options tailored to the participant’s individual needs. Some of the key services include:

  • Personal Care Assistance – For support with daily living activities like bathing, dressing, and mobility.
  • Transportation – Assistance with getting to medical appointments or community activities.
  • Personal Emergency Response Systems (PERS) – Peace of mind with 24/7 monitoring for emergencies.
  • Consultation Services – Expert guidance to ensure the right services are chosen and implemented.

How CFSS Works

Participants in CFSS can choose between two service delivery models:

  1. CFSS Agency Model – A CFSS provider agency manages the hiring, training, and supervision of CFSS workers on behalf of the participant. This model includes professional oversight and support from the agency.
  2. CFSS Budget Model – In this model, participants have control over their care, hiring their own support workers, and managing their own budget with assistance from a Financial Management Services (FMS) provider.

Both models aim to provide maximum flexibility, allowing individuals to choose the services that best meet their needs.

Provider Enrollment and Responsibilities

CFSS services are delivered through a network of qualified providers. To ensure the highest level of care and support, we work with:

  • Home health agencies
  • CFSS provider agencies
  • Financial management service (FMS) providers
  • Consultation service providers

Our providers are carefully selected and trained to ensure they meet Minnesota Department of Human Services (DHS) standards.

The Role of Lead Agencies

Each CFSS participant works with a lead agency (county, tribal government, or managed care organization) to complete an assessment. The lead agency evaluates the participant’s needs and determines the level of care required. Once the assessment is complete, the participant can choose their preferred service delivery model and start receiving support.
